First Visit Instructions

If you have completed your online paperwork please arrive 20 minutes early to fill out any additional paperwork and allow for proper check in that may be needed. Please arrive 30-40 minutes early if you have not completed the online paperwork. If you arrive late, your visit may be cut short.

Please take some time to thoroughly complete the paperwork so the therapist has the best information about you and your condition.

Please wear something comfortable and appropriate for your condition. For example, if you are being seen for your knee please bring shorts.

We encourage you to read up on our team page so you better know the therapist you will be seeing.

If applicable, please bring payment, we currently accept credit cards, cash or check.
