First Visit Instructions

We’re glad you’ll be joining us. To make your first visit smooth and effective, please review the information below.

Arrival time

• If you have completed your online paperwork, please arrive 20 minutes early to complete any additional forms and allow time for check-in.
• If you have not completed the online paperwork, please arrive 30–40 minutes early.
• If you arrive late, your visit may be cut short to avoid delaying other patients.

Paperwork

Please take the time to complete all paperwork thoroughly — the more information we have, the better your therapist can assess and treat your condition.

What to wear

Wear comfortable, loose clothing appropriate to the area being treated. For example, bring shorts if you’re being seen for a knee or thigh issue.

Payment

If applicable, please bring payment. We currently accept credit cards, cash, and checks.
